Most customers use a subdomain (surveys.yourcompany.com) and add a CNAME. Use an A record only if you’re pointing the apex (yourcompany.com), since DNS does not allow CNAMEs at the apex.
Name value doesn’t have your domain appended twice (e.g. _vercel.surveys.yourcompany.com.surveys.yourcompany.com). Most DNS providers auto-append the apex. The domain is registered at the organization level, but each team within the organization must be explicitly granted access before its members can publish surveys at that URL. To grant access, open the domain card, pick the teams in the dropdown, and click Save. Revoking is the same flow — remove the team from the selection.
